Support Letters:
*The Dean’s letter will be inserted into your final packet by the Dean’s office. You may leave a placeholder(s) for the page(s) if you like. All other support letters must be inserted before submitting your final packet.
* Chairs letters are highly regarded by the committee members. It is important that Chair letters show strong support for the candidate.
* Address letters of recommendation to “Award Name Selection Committee”

CIS Comments:
Nominees are to submit all CIS written comments from the last two years of teaching. For spring awards you may include Fall CIS comments if available. All comments must be scanned from the originals and saved into a single PDF.

The university-wide teaching awards selection process is an annual holistic assessment that evolves with each new selection committee formed from faculty across the disciplines. The committees are instructed to review the chartered guidelines for each award with the autonomy to then create an assessment matrix with CIS scores potentially being one component of the assessment. What we seek with the infusion of new faculty and a freshly interpreted holistic selection processes is that awards will reflect the evolution of teaching excellence in its many diverse forms on campus.

*Use the template provided by the Provost office: Course Data Sheet Template
*The Provost’s Office will complete the grade data highlighted in yellow.
*Return form to sharon.williams@austin.utexas.edu. The form will be completed by the Provost office and returned to you in a timely manner.
*For spring awards you may include Fall courses.
